The subacute program at The Elms offers intensive rehabilitation in a setting that is conducive to the healing process. Our state of the art gym is open 6 days a week and is well equipped to meet all of your rehabilitation needs, including a fully functional kitchen that is designed to enable clients to practice the skills in the kitchen to return home independently.

Our rehab team consists of a board certified physiatrist, physical therapists, occupational therapists, and a speech language pathologist who is certified in VitalStim therapy. This team, in conjunction with the medical attending, professional nursing staff, registered dietician and social services director, works together to identify your needs and create a program to maximize your ability to lead an independent life.
In addition, we believe that the continuity of care that you receive from our in house therapy department ensures that we can assess and identify changes in your functional level as quickly as possible, thereby enabling you to more quickly achieve your goals.
